Family Nurse Practitioner - Unified Primary Care Behavioral Health

Eugene, Oregon

PeaceHealth is seeking a 0.8-1.0 FTE Family Nurse Practitioner to join our Unified Primary Care Behavioral Health (UPCBH) team in Eugene, Oregon. This is an exciting opportunity to practice within an integrated care model that brings together primary care and behavioral health services to support whole-person wellness.

As part of a collaborative multidisciplinary team, you will provide primary care services for patients with complex behavioral health and medical needs while partnering closely with behavioral health clinicians, psychiatrists, addiction medicine specialists, care coordinators, and community resources. This role offers the opportunity to make a meaningful impact on a vulnerable patient population while working within a highly supportive and mission-driven environment.

Position Highlights
• 0.8-1.0 FTE with flexibility in scheduling
• Integrated role within PeaceHealth's Unified Primary Care Behavioral Health program
• Collaborative, multidisciplinary team including primary care, behavioral health, psychiatry, addiction medicine, and care coordination
• Opportunity to provide whole-person care for patients with complex medical, behavioral health, and substance use needs

Compensation & Benefits
• Salary Range: $160,992.00 - 187,616.00 DOE

Requirements
• Board Certified Family Nurse Practitioner (FNP-C or equivalent)
• Oregon NP license, or ability to obtain
• DEA registration, or ability to obtain
• Experience in primary care preferred
• Interest or experience working with behavioral health and/or substance use disorder populations preferred
